I acknowledge that even with the best coaching, use of the most advanced protective equipment and strict observance of rules, injuries are still a possibility. On rare occasions these injuries can be so severe as to result in total disability, paralysis, or even death. We the undersigned, acknowledge that we have read and understand all aspects of this form and grant permission and consent as required. We the undersigned, agree to abide by good sportsmanship and all Illinois Thunder Basketball rules. *

I/We the parents or guardians of the child listed above, hereby give my/our approval for him/her to participate in any and all Illinois Thunder, NFP Basketball activities. I/We assume all risks and hazards incidental to such participation including transportation to and from the activities; and I/We do hereby waive, release, absolve, indemnify and agree to hold harmless, the local Illinois Thunder, NFP Basketball League, Illinois Thunder, NFP, the organizers, sponsors, supervisors, participants and persons transporting my/our child to or from activities, for any claim arising out of any injury to my/our child, whether the result of negligence, or for any other cause, except to the extent and in the amount covered by provided accident or liability insurance. I hereby give my consent, in the event of injury or illness, for emergency medical treatment as may be necessary for the welfare of the above named youth, by a physician, qualified nurse, certified athletic-trainer, and/or hospital during all periods of time in which the youth is away from his/her legal residence as a member of a recognized Illinois Thunder, NFP activity team or group. Further, I/We hereby waive, on behalf of myself and the above named youth, any liability of Illinois Thunder, NFP, its agents or board members, arising out of such medical treatment. *

The essential elements of character-building and ethics in sports are embodied in the concept of sportsmanship and six core principles: trustworthiness, respect, responsibility, fairness, caring, and good citizenship. The highest potential of sports is achieved when competition reflects these "six pillars of character." I therefore agree: 1. I will remember that children participate to have fun and that the game is for youth, not adults. 2. I (and my guests) will be a positive role model for my child and encourage sportsmanship by showing respect and courtesy, and by demonstrating positive support for all players, coaches, officials and spectators at every game, practice, or other sporting event. 3. I (and my guests) will not engage in any kind of unsportsmanlike conduct with any official, coach, player, or parent such as booing and taunting; refusing to shake hands; or using profane language or gestures. 4. I will teach my child that doing one's best is more important than winning so that my child will never feel defeated by the outcome of a game or his/her performance. 5. I will respect the officials/coaches, and their authority during games, and will never question, discuss, or confront coaches at the game court, and will take time to speak with coaches at a mutually agreed upon time and place. 6. I also agree that if I fail to abide by the aforementioned rules and guidelines, I will be subject to removal from the sporting event. *
